Medicaid Home Care Options in New York
New York Medicaid supports several types of home and community-based care. The appropriate option depends on a person’s Medicaid eligibility, medical needs, functional abilities, approved service hours and care preferences.
Common options include agency-provided personal care services, Managed Long-Term Care and the Consumer Directed Personal Assistance Program.
Personal Care and Home Health Aide Services
Through agency-directed home care, eligible individuals may receive support from trained Personal Care Aides or Home Health Aides.
Depending on the approved plan of care, assistance may include:
- Bathing, dressing and grooming
- Mobility and transfer assistance
- Meal preparation
- Light housekeeping
- Medication reminders
- Companionship and supervision
- Support with other activities of daily living
With agency-directed care, the home care agency recruits, screens, schedules and supervises the aide. This option may be appropriate for individuals and families who want an agency to manage caregiver staffing and ongoing service coordination.
The Consumer Directed Personal Assistance Program, commonly called CDPAP, is a New York Medicaid program for eligible individuals who want greater control over who provides their care.
Under CDPAP, the consumer—or an eligible designated representative—generally takes responsibility for recruiting, selecting, training, scheduling and supervising personal assistants. Depending on program rules, a personal assistant may be a trusted friend or qualifying family member.
New York now uses Public Partnerships LLC, or PPL, as the statewide fiscal intermediary for CDPAP. PPL handles fiscal-intermediary responsibilities such as consumer and personal-assistant registration, payroll processing, required documentation, timekeeping systems, compliance and customer support.
Sincere Care Agency is not the statewide fiscal intermediary and does not replace PPL’s role. Our team can provide general information about the differences between CDPAP and agency-provided home care and help families understand which care model may better align with their needs. Questions involving CDPAP registration, personal-assistant payroll, PPL accounts or timekeeping should be directed to PPL or an officially approved CDPAP facilitator.
CDPAP and Agency-Provided Home Care: What Is the Difference?
CDPAP gives an eligible consumer significant responsibility and control over personal-assistant services.
The consumer or designated representative generally:
- Recruits and selects personal assistants
- Trains and supervises personal assistants
- Schedules approved care hours
- Reviews and approves time worked
- Works with PPL for registration, payroll and administrative requirements
Agency-Provided Home Care
With agency-provided home care, a licensed home care agency manages much of the caregiver process.
The agency generally:
- Recruits and screens aides
- Coordinates scheduling
- Provides oversight and support
- Helps address staffing changes
- Delivers care according to the approved plan of care
Neither model is automatically right for everyone. The better choice depends on how much responsibility the consumer or family wants to manage, whether they already have a preferred caregiver and what services have been authorized.
Managed Long-Term Care
Managed Long-Term Care, or MLTC, helps coordinate long-term services for eligible Medicaid recipients who need ongoing care while remaining in their homes and communities.
An MLTC plan may coordinate services such as:
- Personal care assistance
- Home health aide services
- Skilled nursing
- Therapy
- Medical equipment and supplies
- Other covered long-term care services
The MLTC plan, local social services district, or other responsible Medicaid entity determines eligibility, conducts or coordinates required assessments, and authorizes covered services.
