Medicaid Home Care Programs in New York
The current Medicaid programs that support home care services include:
Consumer Directed Personal Assistance Program (CDPAP)
CDPAP provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services. Services can include any of the services provided by a personal care aide (PCA), home health aide (HHA), or nurse. Recipients have flexibility and freedom in choosing their caregivers. The consumer or the person acting on the consumer’s behalf (such as the parent of a disabled or chronically ill child) assumes full responsibility for hiring, training, supervising, and – if need be – terminating the employment of persons providing the services.
We work as a Fiscal Intermediary which means that once you are approved for the CDPAP program you can utilize our services to be the provider of wages and benefit processing for your Personal Assistant. You will be responsible for the hiring and supervising of your Personal Assistant. This is a great way to have someone to take care of you that you feel comfortable with and who is your choice.
Managed Long-Term Care (MLTC)
The Managed Long-Term Care (MLTC) Program is designed for Medicaid recipients in New York who require long-term care services at home due to aging, disability, or chronic illness. Instead of receiving home care directly from Medicaid, beneficiaries must enroll in an MLTC insurance plan that coordinates and pays for their care. MLTC covers services such as personal care assistance, home health aides, skilled nursing, physical therapy, and durable medical equipment. The goal of the program is to help individuals remain in their homes and communities rather than being placed in nursing homes or institutional care settings.
Personal Care Services Program (PCS)
The Personal Care Services (PCS) Program provides Medicaid-covered assistance with essential daily living activities for individuals who need help due to illness, disability, or frailty. Services include bathing, dressing, grooming, toileting, meal preparation, and light housekeeping. Unlike MLTC, PCS is a more straightforward Medicaid benefit, allowing individuals to receive in-home assistance without enrolling in an insurance plan.

How to Apply for Medicaid Home Care in New York
2. Apply for Medicaid: Through NY State of Health or the NYC Human Resources Administration (HRA).
3. Get an Assessment: A nurse or care manager will assess your needs to determine eligibility.
4. Choose a Provider: Enroll in a Medicaid Managed Long-Term Care (MLTC) plan or another program that fits your needs.
